What to Expect on Your First Visit

We want every dental visit to be a great experience, starting with your first one.

What to Do

Please arrive 15 minutes prior to your scheduled appointment and plan to spend between 60 – 90 minutes in our office. You will receive a comprehensive oral examination and x-rays to help your dentist assess your oral health. Your dentist will look for signs of:

  • Tooth decay
  • Oral cancer
  • Bone loss
  • Periodontal disease
  • TMJ disorder
  • Mal occlussion

What to Bring

To save time, please bring with you:

  1. dental insurance card, if applicable
  2. photo ID card
  3. completed new patient forms

New Patient Forms

We will need information about you to get started. Please download and complete the forms below and bring them to your first visit. If you are not able to complete these forms prior to your first visit, please arrive 15 minutes before your scheduled appointment.


Your Treatment Plan

Your dentist will customize a personal treatment plan for you based on the clinical findings of your initial examination. It will specify the type of cleaning and additional visits you need and include recommendations for home care products. Your dental team will review your treatment plan options and begin treatment after obtaining your consent.

Cancellation Policy

We set aside dedicated time in our office for your dental appointment. If you find it necessary to cancel, please provide 24-hour advance notice. Without proper notice, you may be charged a $25 fee.
